TBCC Pre-Registration Form
Please fill out this short pre-registration form to assure that I have the correct information to
contact you and to verify the names and current placement of singers in the chorus.
The primary contact should be:
1) A legal guardian of the singer
(If you are the contact for the singer but not the legal guardian, fill out the form but please let me know via email)
2) The person who is to receive information from TBCC for the singer
3) The person who will check for emails from TBCC of a regular basis (at least twice a week)
4) The person who will also regularly log in to the TBCC member’s area to update profile information and check for new information posted there.
